Format of Amethyst 1 template CSV file for import

Each line represents one question.
Format : Each field must be separated by a pipe character "|"

Fields

  1. Template Name (max 30 chars)
  2. Category Name (max 30 chars)
  3. Question
  4. Sequence #

This is the sequence the questions must display inside its category.
NOTE! No Duplicate Sequence numbers allowed with in a category.

  1. Answer type (Value can be 0,1 or 2)

If this is a positive category:
0 = Yes/No
1 = Yes/Need Attention/No
2 = Poor/Below Average/Average/Above Average/Excellent.

If this is a negative or total fail category:
0 = leave this field 0 for all questions.

  1. Weight in Category (1-9999)

If this is a positive category, this is the number of points this question counts within the Section.

  1. Comments (max 50 chars)
  2. Category type (Values are 0,1 or 2)

0 = Positive category
1 = Negative category
2 = Total fail category.

  1. Negative total

If this is a negative category, this is the total marks for the entire category.

  1. Need score

If this is a negative category, this is the number of points to deduct for a Needs Attention answer.

  1. Poor score

If this is a negative category, this is the number of points to deduct for a Poor answer.

An example would looks like this:
